Concordia Dental Plan: Options

For employers, Concordia dental plan options include:

  • Concordia Flex is an employer-funded passive PPO plan that is available for businesses with as little as two employees. Employers can avail of over 9,000 program options with further customization available for organizations with more than 200 enrolled employees.
  • Concordia Preferred is an employer-funded active PPO plan that offers covered individuals the freedom to see any dental care provider, but will reward them with significant cost savings when they use an in-network dentist.
  • Concordia Plus is a dental HMO plan that requires covered individuals to nominate a primary dental care provider who will coordinate all their care. The plan covers all eligible preventive and diagnostic services with little or no copayment and no annual maximums and no deductibles.
  • Concordia Choice is a PPO plan that offers a wide variety of affordable and flexible program options as well as low minimum participation requirements. Discounts on eye care services are also offered through Davis Vision.
  • For individuals, Concordia dental plan options include:
  • IND 100 offers 100% coverage for preventive and diagnostic dental services but does not cover basic and major services. It has no annual deductible and no annual maximum per insured person.
  • IND 200 has similar benefits as IND 100 but charges an annual deductible of $50 per insured person and an annual maximum of $2,000.
  • IND 300 covers diagnostic and preventive services, as well as basic services, for up to 50% but does not cover major services. There is an annual deductible of $50 per insured individual and annual maximum of $1,500.
  • IND 400 offers 100% coverage for diagnostic services, 80% for basic services and 50% for selected basic services and major services. A $50 annual deductible is charged per insured individual and $2,000 annual maximum.
  • IND 500 charges the same deductible and has the same annual maximum as IND 400 but offers 60% coverage on selected basic services and 60% on major services (after a six-month waiting period). Diagnostic and preventive services, however, are not covered.

United Concordia Dental Insurance Products

The coverage available through this insurer may vary from one state to another, and will also depend on the policies selected by companies to offer their employees.

Here is a list of some of the products offered through United Concordia:

  • Concordia Flex: This coverage provides fee-for-service dental insurance that is available to companies regardless of the number of employees they employ. It does not include a network or preferred providers if the Indemnity option is chosen; otherwise, policyholders can utilize the dental PPO option and receive discounts for visiting in-network dentists.
  • Concordia Choice: This coverage provides dental coverage to employees; however, the employees may be fully responsible for paying for the coverage as there is no minimum contribution requirement from employers necessary to offer this coverage. This product is a dental PPO and can also include a vision plan.
  • Concordia Access: This policy combines a dental PPO with a dental discount plan. Utilizing an in-network dentist will result in no charge to policyholders for covered dental procedures and discounts for procedures that are not covered by the policy.
  • Concordia Preferred: This is a standard dental policy that provides greater coverage benefits when the policyholder utilizes in-network dentists. This plan is a dental PPO.
  • Concordia EPO: This policy requires no referrals for care and is available to businesses with ten or more employees. This dental PPO is only available in a limited number of states including Washington and Oregon.
  • Concordia Plus: This policy is a dental HMO. There are no coverage benefits provided when the policyholder visits a dentist who is not within the Concordia network. This policy offers a broad range of coverage for preventative dental care.
